Scholarship Application

2008-2008 Scholarship Application- $1,000

Requirements for Application:

1) The applicant must have been accepted by, or currently enrolled in an accredited college or university.
2) A parent or legal guardian of the applicant must be a paid member or associate member of the Dallas Claims Association.

Attachments:

1) A copy of the applicant’s high school or college transcript(s) must be attached.
2) A copy of the applicant’s letter or acceptance must be attached (high school seniors).
3) A copy of any awards, recommendation or documents that will assist in the selection process for the committee.
4) One (1) personal recommendation and two (2) academic recommendations. Must be original, no photocopies.

Guideline Agreement

I understand that any money awarded to me will be paid directly to the accredited institution I have chosen to attend. I must enroll for a minimum of six (6) college academic (or vocational equivalent) per semester, use the funds within one (1) calendar year and maintain passing grades in the first semester in order to receive funds in the second semester. This money will be used for tuition, books, fees, dorm room and other items related to enrollment in the institution.

I understand that if I do not enroll or if I withdraw from the school, any unused money will revert to the Dallas Claims Association Scholarship Fund.

This application, an official transcript and three recommendations forms are required for consideration of the Dallas Claims Association Scholarship Fund.
 
Deadline for submission of the material is July 1. Applications received after this date will be reviewed on an individual basis.
